I don't know if this describes your problem, but you might give it a try. There has been a problem for a while that sometimes the microphone stays on when you use Siri. If this is the problem, hold the Home button down until you hear the 2 beeps from Siri. Then release the button. Then press the Home button again to close Siri.

If you haven't done so already, add the volume setting to your rotor. You can find that in the Settings app, under General/Accessibility/VoiceOver/Rotor. After that, turn to volume in your rotor and do a one-finger flick up. This will raise your VO volume. Then use your volume down button to lower your VO sounds.
