Hello Andrew, I too am unable to reproduce your findings with VoiceOver on an iPhone5s running IOS 9.0.1. I did have this problem once though and at the time, restarting the phone by powering it all the way off and back on resolved the issue for me. Have you tried this? I know that it might be an obvious step, but sometimes this does resolve problems.
