I'm not sure when I'll get to this. Turns out I need to work this weekend.
BTW, I've never downgraded on this phone. When I installed IOS 9, I did
a complete restore and then restored the last backup I had taken just
before the upgrade. I moved up to the IOS 9.1 beta but then just moved
up to the released version of IOS 9.1. Like I said, I never down graded.
I've also been having this problem since I initially upgraded to IOS
9.0, and I've seen it consistently on both my own iPhone 4S and my
employer's iPhone 5.
I will give what you suggest a try though, since I do find this bug so
frustrating.

Hello Christopher,
I completely understand your pessimism on this process, in fact, I shared it
but, I can testify that it works.
The odds of things kind of going hay-wire are greatly increased if you
downgraded or switched to a public release on a device that was used in a beta,
without using the method I posted.
Remember, the key here is to set the device up, complete with activation, as a
new device. Just so you know, since you intend to eventually restore your
device from a backup, when setting up the phone as a new device, you can skip
many of the setup steps such as entering WiFi passwords, setting up iCloud,
etc. All of these items will be restored upon restoration from the backup.
The only thing you want to do is to receive an incoming call on the newly
restored phone, with a headset attached, in order to verify that the problem
has been resolved.
Now, let me add one more ingredient to the troubleshooting soup, as it were
(smile).
While highly unlikely, it is possible that you downloaded a corrupted version
of iOS 9.1. I know Apple loves to claim that such things don't happen but,
take it from me, they do, albeit rarely. Therefore, as a last resort, should
the steps I posted do not resolve your issue, then you will want to repeat the
process only first have iTunes re-download a fresh copy of the .IPSW file,
before installing.
Before I performed the steps previously posted, I chose to force iTunes to
download a fresh copy of the .IPSW file, before I began. I like to always plan
for the worse-case scenario (smile).
In order to force iTunes to download a fresh copy of the .IPSW file, all you
need do is to delete the current copy from your computer, before launching
iTunes.
The location of the file differs between the Mac and Windows so you can Google
the file location or, if you are using Windows, let me know and I'll look up
the exact folder location for you.

I'll give this a try this weekend, but I did restore my phone when I upgraded
to IOS 9. I'm also running IOS 9.1, and I participated in the IOS 9.1 beta
since I didn't figure I had anything to lose. I'm not optimistic this will
solve the problem, but I'll give it a try.

Hello Christopher,
If you have confirmed that you have, indeed, updated your device to iOS 9.1,
and you are still continuing to experience the VoiceOver incoming call bug,
which occurs when the included Apple wired headset is attached, then you will
most likely need to perform a fresh install of the OS. To do this, you will
need access to a computer running the latest version of iTunes.
Assuming you have such a computer, then perform the general broad steps as
follows:
1.
Sync your device to iTunes on your computer. During this process, a backup of
your device will be made. I also suggest that you make certain that your
backup is encrypted.
2.
Restore your phone, via iTunes. At the appropriate prompt, select to setup
your phone as a new device. Please make certain that the phone has an active
SIM card installed.
3.
Once the phone has been setup as a new device, receive an incoming call, with
the attached headset to make certain that the problem has been resolved.
4.
Finally, restore the backup, which was made in step 1, back to your phone.
Remember, restoring from a backup, even an encrypted one, does not install the
OS but merely the settings and the apps/app data.
Good Luck,
Mark

I still have the phone issue where I can't answer a call with my headset
attached or I lose VoiceOver.

Hi Mario,
Applevis stated that they have not yet completed testing and people should let
them know if other bugs have been fixed. One bug which I think was a big deal
for those who were effected was the phone issue where when a headset was
connected your iPhone could become totally unresponsive with Voiceover gone
silent after hanging up a phone call. As far as I can tell and from what I
heard from others that has been fixed. Also, the focus bug has been fixed or at
least it is fixed in apps like mail where focus now correctly remains on
whichever email you were on when you open a message and go back out to the
inbox.
I can't speak for any Bluetooth issues since I very rarely use a Bluetooth
keyboard, but for the most part I think iOS 9.1 is now every bit as good or
better than the latest release of iOS 8 was and there is definitely a huge
improvement in how much time it took to fix some of the bugs. After all, we are
now just a bit more than a month past the date when iOS 9 was released
publically, with iOS 8 it took a lot longer for some of the major issues to be
fixed.
I have no problem criticizing Apple if they drop the ball on something, but I
think so far iOS 9.1 seems to run very stabil, it's responsive and there are no
battery drain issues. Apple needs to be reminded about bugs which persist, but
I don't think it's justified to totally bash them for putting out a real crappy
version of iOS since especially 9.1 is far from crappy.

Sieghard

a shame.
I do not want to believe it.
I thought that Apple had changed their irresponsible attitudes toward the blind.
apple how you're going to justify this lack of irresponsibility that you have
had in recent times with the blind?
only 5 bugs have been fixed.
and the other 20 in the most serious categories?
a shame
These were the bugs fixed.
Accessibility Bugs Fixed in iOS 9.1
list of 5 items
• In the Mail app, VoiceOver now states the actual number of messages in a
grouped conversation rather than saying “multiple messages”.
• Audio now returns to the correct level after being interrupted by VoiceOver
when audio ducking is enabled • Fixed the problem where VoiceOver behaviour
appeared to indicate that it was not possible to enter text into some webpage
text fields (such as the Search fields on the AppleVis and BookShare websites).
• VoiceOver sound effects no longer randomly disappear.
• Switching VoiceOver voices in Settings > General > Accessibility > VoiceOver >
Speech > Default Dialect now behaves as expected (the only situation where voices may not
switch automatically are when you have limited storage space available on your device or poor
internet connectivity).
