It compresses all the apps on your home screen into the bottom half or maybe a bit more than half of your phone. You activate it by tapping twice on the home button, that is just touching it twice, not actually pressing it. You will hear a fairly unique sound which indicates reachability has been activated, if you don't do anything for a few seconds you hear the sound again and everything is back to normal, the effect does not last long if you don't touch anything and pressing the home button stops it instantly.
If you want to turn it off, it is under Accessibility Settings.
