TuneIn Radio works for me. It does everything you mentioned below and is Voiceover accessible.
I have a list of favorites/presets that include some general music stations, some of my local Southern California stations and local stations from Guanajuato state, León GTO and Berlin/Brandenburg Germany. There are other radio apps out there, but really, at least in my case, TuneIn radio does everything I want and does it well enough that I haven’t really bothered playing too much with other apps. There’s a free and paid version of TuneIn Radio. I’m currently using the free version.
