Hi,
My brief testing on both Windows 10 and iOS shows quite a potential
(iOS version isn't polished yet and you'll need a Microsoft Account to use it).
For Cortana on desktop: I think there is a better place to discuss
this sort of thing:

I'll answer as to why Cortana nor other screen readers read
reminders, as well as other accessibility considerations for Cortana
and Windows 10 on that forum (not here).
Cheers,

Subject: windows 10 personal assistant on a desktop computer
Here are a few more comments from a screen reader using
friend on how Cortana works on a desktop computer. I don't know how
this compares with it's delivery on an IOS device.
If you give Cortana a reminder, when the reminder time arrives it
puts the reminder message on the screen, but does not read aloud
what the reminder message is. It will open word pad, but my friend
did not succeed in getting it to open documents or even internet explorer.
What he likes is that you can ask cortana what time it is, what the
date is, to tell you a joke or sing you a song, or tell you a
story. You can ask it silly things, like do you love me, or what is
your favorite color? If you ask her who her parents are, she says,
"bill gates."
you can ask Cortana things like, convert quarts to liters, or how
many teaspoons are in a tablespoon, or how many miles equals how
many kilometers, or just about any conversion you want. If you ask
how old jimmy carter is, she will give the correct answer. If you
ask her how old john denver is, she will reply with something like
john denver was 42 when he died in 1987.
These are not the correct numbers - I just made them up by way of example.
Cortana can also give you the dictionary's version of word
definition, you say hey cortana to wake her up, then you say,
define, and then say the word, and she's right on. And if you have
some music in your documents folder, say maybe some beatles songs,
ask cortana to play i wanna hold your hand, and she will!
if cortana can't give you a quick and easy answer she will open a
website that will probably help you find the answer you need!
According to my friend who I asked about this, another important
note which might help some is that if you get windows ten, there is
currently no screen reader that works good with windows ten. By
default windows ten will use a new version of a browser, it it is
called edge, to fix this press the windows key, then type in default
programs, on the next screen you will be able to change the default
back to internet explorer. My friend recommends doing this, since he
finds that all the screen readers he uses work much better with
internet explorer than with the edge browser!
It seems to me that Cortana is more a benefit than not.
