I wouldn't think this particular holder would work with an Otter box case, but maybe someone who actually has it could answer this question or you could contact support for this particular holder.
I do believe I saw a discussion about another manufacturer who would make custom cases/lanyards that work with other cases, but I don't have that info myself.
