Just checking ... Could you be using the speech off jesture accidentally? You can test for this by telling Siri to turn voice over on; if it's already running but not speaking, Siri will tell you that it's already on. If it has stopped running, then Siri will start it speaking again, and you'll at least know it crashed.

I've run into a few apps which seem to disable Voice Over when you make them active, and then I would do a quick double press of the home button to go to the app switcher, and Voice Over would return (have you tried this?).
