If the screen is clean and all that and it is so unreliable then you can try a 
full restore and set up as a new phone, if that doesn't fix it then it 
definitely sounds like a defect and should be covered under Apple's warranty 
even if you didn't buy Apple Care Plus. You say the phone is a 6S so even if 
you got it on day 1 it's only a bit over 7 months old and it does have a 1-year 
warranty.

Yeah,
I have never used any type of screen covering, and the glass is clean, and I
just have a very basic case on it that covers the back of the phone.
It has never been dropped or dunked into any kind of water etc.

Clean it off with a dry tissue,  grease smears would do that.  So would a
loose screen cover of a case that otherwise completely 
covers the phone. But you'd feel the ripple of the cover under your finger
if it was that.

Ok,

Getting frustrated. The touch screen on my phone doesn't seem to work
constantly anymore.

I have toggled VO off and on, turned off the phone and back on.

I have hard core rebooted the darn thing etc.

When I slide my finger across the screen, it is only lucky once in a while
it works. I can't answer calls, I can't get into the phone, nothing seems to
work.

Any suggestions?
