You must press the speakerphone button after the call has started. Skype does not utilize the nearness sensors like Facetime does. Skype requires VoiceOver in reduced volume mode before pressing the speaker button. If you want the iPhone to automatically switch to speaker when you move the phone away from your ear then you need to use Facetime, if the person is another Apple user.
