The effects I am pretty sure are not accessible at this point. A
sighted person would I believe tap and hold on the Send button
and then be able to choose the effects, but for Voiceover users a
double tap and hold does nothing until you let go and then it
sends the message.
As for emoji predictions, they do work. Try and type "Hi" in a
message and then check the predictions above the keyboard, when I
just did this one of the options was a "waving hand emoji" or
type "Love" and you get a "red heart emoji".

You can only add effects to iMessages. I know, sucky for those
who don't have iMessage, or all of us who have friends who just
happen to not be on Apple devices, but yeah. I can't figure out
how to emojify words and such, emoji replacements for words in
messages where words can be replaced by an emoji.
