You have to double tap on the one you want and then find the Send button.

I generally prefer to leave the motion off becaue it slows down performance with some apps so it's too bad there's not an easy way to turn it off and on while you send a message. Turn off reduce motion in accessibility settings. Now, after you type your message, double tap and hold on the send button and the effects screen will pop up. I just tried this and it works beautifully. I sent a message and voiceover even announced the effect I had chosen. Now, if only there was a way to make digital touch features more accessible, like if someone sends you their heartbeat, it would be nice if VO could announce that. Obviously, it wouldn't be able to interpret a sketch or handwriting, but if it said sketch, heartbeat, tap, etc, at least then we'd have a better idea what was being sent.
