Make sure you’re not inadvertently doing a 3-finger double tap (which turns off 
speech, but doesn’t turn off voiceover) when you intend to use a 2-finger 
double tap to hang up from a call or pause media playback. I can’t tell from 
your message if these are things you’re doing before you turn off you phone, 
but if they are, it could the culprit.

Subject: unpredictable loss of voice over

Hi all.  I’m having an issue with loosing voice over.  I will use the phone 
normally, and the next time I unlock the screen there is no voice over.  The 
sounds are all still there. When I ask Siri to turn it on he says “voice over 
is already turned on.”  I do a  3  finger double tap in case speech is turned 
off which says nothing, so I do it a second time, still nothing. When I hold 
the home button and lock screen buttons down for ten seconds to re-boot the 
phone voice over comes back and works normally. I can’t figure out what 
triggers the voice over loss or predict it.  It’s happened twice in the last 
day, and a couple of times spread a few days apart.  Has anyone else noticed 
this or is it just my phone.  I didn’t have any of this until I updated to IOS 
10.02.  I’m running an IPhone 6.  Thanks.
