Make sure you’re not inadvertently doing a 3-finger double tap (which turns off speech, but doesn’t turn off voiceover) when you intend to use a 2-finger double tap to hang up from a call or pause media playback. I can’t tell from your message if these are things you’re doing before you turn off you phone, but if they are, it could the culprit.

I’m having an issue with loosing voice over. I will use the phone normally, and the next time I unlock the screen there is no voice over. The sounds are all still there. When I ask Siri to turn it on he says “voice over is already turned on.” I do a 3 finger double tap in case speech is turned off which says nothing, so I do it a second time, still nothing. When I hold the home button and lock screen buttons down for ten seconds to re-boot the phone voice over comes back and works normally. I can’t figure out what triggers the voice over loss or predict it. It’s happened twice in the last day, and a couple of times spread a few days apart. Has anyone else noticed this or is it just my phone. I didn’t have any of this until I updated to IOS 10.02. I’m running an IPhone 6. Thanks. God bless!
