In the app, you can either open a picture from your camera roll or take one from within the app. Within the settings, you can even change the mode to either have a generic description, detailed description or text. When selecting a mode, if you swipe to the right, there is a description of what the mode does and how long it takes for the photo to upload and return results. So that is most likely the reason why it may have taken a little while before it read the description.
The app is layed out very simply, the top left corner is a Settings button, the top right is an Instructions button, that when pressed a built in voice reads out the instructions on how to use the app. The bottom left corner is to choose a photo from your camera roll, and it appears you can even select a photo from your drop box as well.
