The site where it happened this weekend was a site that probably has it's own accessibility issues: www.megabus.com I was trying to fill out the date field, and after writing a few numbers in the date, the field disappeared. This was one where when my wife looked without VoiceOver, she had some issues herself. Though the keyboard did not disappear for her, it was not obvious how to buy tickets from that site. I will keep watching this behavior to get some sort of pattern. I don't use Safari all the time, but I do use it fairly often.
