Once KNFB Reader starts reading, there is a speed adjustment button on the lower part of the screen. A two finger double tap will pause the speech, then you could adjust the speed for that document. Also, on either side of the play button, is a button for previous and next sentence. Or, you can touch the screen and find the place you want to read from and use VoiceOver to read it, but that is awkward at best.
Also, if you go into settings, in the Reader section, you can set the default speed, you could also turn off automatic reading so you would have to hit the play button to start it reading or just use the next sentence button to read one sentence at a time. You could also turn off the read in background setting so it won't keep reading when you switch to another app or just hit the home button.
