Do any of you sophisticated programmers or techs know of a Web survey format that is fully accessible? I try to take a local library survey that was done on monkey and though the checkboxes could be toggled with a single finger double tap once the rotor was turn to form fields, the edit fields would not come alive and I kept getting flip from one question to another. Then there were headings that seem to be double headings which made no sense at all whatsoever. Just cumbersome and wordy. The library reference people that deal with accessibility issues asked if I knew of a Web survey format that was fully accessible that they could look into. Does anyone have a recommendation?
