Hello Neil, I just performed several tests to see if I could reproduce your volume issue but I cannot.
Regardless of whether I initiate or answer a call, when the call ends, either via a double-tap on my part or the other party disconnecting, VoiceOver acts just as it should, or rather, as it always has. In short, I cannot reproduce your problem on my iPhone 4 S.
