Sadly, the one app that easily did that is not being updated and won't work beyond iOS 10, and is sketchy even then. List Recorder Full, when set to use M4A format could be appended to. You might try creating a grocery list with Siri and then just say add peanut butter to my grocery list, etc. Then, when you are done, you can either have Siri read it, oor I believe just open it so you can flick through it. Hi, Audio Memos Pro will let you merge two files. Or, more if you wish. Once you have your files recorded that you want to become one file one after or before the other, you hit Edit, then double tap on each one, then find the merge button toward the bottom and activate it. Then, each part will show its beginning (called left) and its ending (called right) and then there will be a reorder button next to it so you can double tap and hold on that and slide it before or after the other file. Once you activate the merge button, they will be combined and the file name will have each file within it with plus between them so you know what is contained in that merged file.
