First of all, it’s more like at least 8 seconds and yes, auto dial is turned 
off by default.
On the other hand Apple has made this available after pressing the power button 
5 times quickly and I really don’t see any chance at all where you would 
accidently press the power button 5 times quickly enough to trigger this, it 
takes a pretty deliverate effort to do this. I definitely turned this on 
because in a pinch that is something I can do with one hand and even if the 
phone is in my pocket and I definitely like this extra piece of mind.

Yes it's turned on by default and can't be turned off. If you go to settings 
and emergency  S-O-S you can turn auto dial on and off. By default auto dial is 
off which means that after pressing the power button five times you would have 
to confirm that calling emergency services is your intention. If auto dial is 
on it will call emergency services in three seconds after pressing the power 
button five times.
