I have sighted help ... the only instruction which we found to enable the 
screen reader was to hold down a certain key on the remote for several seconds 
(this was supposed to turn on the screen reader).  It did nothing for us 
unfortunately.

You need sighted help to set the TiVo up.  Once you get it to work
though, it is fantastic.
