I submitted a request via their help centre and received a confirmation email, 
here is the address, you could try to send an email directly to that and see if 
it works, but it may not:

The process of submitting a request online (I used the link to their Help 
Centre provided in the “this email is inactive” which you said you also 
received was not too difficult with Jaws.

Hi All,

I am trying to reach lyft, but their help center  contact support form is not 
accessible.  I can’t get passed the little thing after the form fields where it 
asks for your namenumber email etc.  the category picker is not accessible.

Does anyone have an email, or phone?  I just tried 
supp...@lyft.com<mailto:supp...@lyft.com>, but that replies back to me that it 
is no longer active.
