If you turn ringtone volume to 100% and then disable change with buttons it should stay at 100%. If change with buttons is enabled there is a much greater chance that you might accidently turn the ringer volume down if you press the volume down button while it is ringing. I am not sure why your Facetime calls are not ringing, they are tied to the ringtone volume and by default whatever you set as your ringtone is also the Facetime tone. However, you can set a different Facetime ringtone in Settings > Notifications > Facetime, tap on "Sounds. Try and select a different ringtone for Facetime, it should play as soon as you double tap it. If it does get somebody to Facetime you and see if it works. If it doesn't I'd try a reboot of the phone then try again. If it still doesn't work as in regular phone calls ring, ringtones play when you select them in Settings > Notifications > Facetime but they don't play if somebody calls you then it's time for a reset network settings. If even that doesn't fix it it's time to completely reset your iPhone back to factory settings, then before you restore from any backups get somebody to Facetime you and see if it now works, then restore your backup and try again. If it still works then everything should be fine, if it doesn't then you probably have a corrupt backup and you have to set up your phone from scratch. Hopefully it will work long before you even get to the reset network settings, but these are the things you can do to continue to trouble shoot this.
