I think it happens when, for whatever reason, your device looses contact with ICloud for a period of time, then reconnects. It only wants you to re-enter your password, so it can make sure the device that is connecting, using your ICloud account really is you.

After almost a year of saying “not now”, the phone locked up and unless I can get a court order there is no possibility of unlocking it.
