I assume that the sound switches from earpiece to external speaker. One way to confirm this is for her to make several longer phone calls using a headset, if the sound doesn’t cut out then the auto switching is the problem. You can disable this in Voiceover Settings, but then of course if she sometimes wants the call to switch to speaker when she takes the phone away from her ear this will no longer work.
