I was going to suggest to first of all reboot the phone, after that if it still happens turn the phone face down, Hey SIRI will not work if the proximity sensor is covered. If SIRI still reacts when the screen is locked and the phone is lying screen side down there is something else going on, maybe the home button is stuck?

Both lay in wait in the background to pick up your speech at a distance as if you might want to issue a command at a distance. So it's hearing you or are you on your computer or something else going on. since you're not really asking the question directly it doesn't have any clue what you're saying which must be why the poor thing keeps saying it doesn't understand what you mean. Turn off OK Siri or turn off OK google depending on which you've got switched on.
