Sometimes voice-over stops talking for me, too. I also use Alex on an IPhone 7 running 12.1. Never thought to change voices. Will give that a try.

If so try switching it to the Tom voice and that should go away. I have that in IOS 12 when I was using Alex as my voice over voice. So I switched over to Tom and my phone doesn’t do that anymore.
