There is an accessibility shortcut just as on an iPhone where you press the home button 3 times, on the watch you press the digital crown 3 times quickly, but if that was never configured it may not work in which case a sighted person has to turn it on in Settings or you have to start Voiceover on the iPhone the watch is paired to, open the watch app, go to General > Accessibility > Voiceover and turn it on there. As for using the watch it is mostly done by swiping and double tapping just as on the iPhone although you can turn so-called digital crown navigation on which allows you to move through items on the screen by turning the crown instead of swiping right or left.
