I am presuming that this concerns reading e-mail on the iPhone. As far as opening an attachment, locate the attached file name and double-tap on that. If you have an appropriate app on your iPhone, the attachment should open within that app.
So far as saving an attachment, I never gave it much thought so am not able to offer any answers there. By the way, if the attachment is a file type that you do not have an appropriate app to open it it will not open.
