I had that bug when I updated from ios 5 to 6, had to completely format and restore the phone to get rid of it.

Subject: Should re-booting an iPhone 4 running IOS 6 cause loss of VoiceOver?

Under IOS 5, if I re-started my iPhone 4 by pushing the power button and holding down the home button for several seconds, the phone would come up talking. Under version 6, it does not. Tripple-click Home does not bring VoiceOver back. I will not have an accessible phone until I get home tonight and can connect it to my PC. Is this a bug or an unwelcome new feature?
