I also experience this, but for me mostly this happens when I ask SIRI to make a call and use a wired headset and then use the headset remote to hang up the call. But I do know that others experience this as well and some a lot more than me and not only for phone calls but also with Skype calls, Zoom and other Voip apps. It needs to be reported to Apple since it's a fairly serious issue but one which may be hard to pin down.
