They may see it, but it's probably not even registering as they can immediately focus on the message body or whatever is important for them to see. Having said this it still happens a fair bit that when you open an email Voiceover focus isn't in the message body where it should be and that is certainly an accessibility issue and hopefully it will be fixed again in a later version. There is no reason why Voiceover should first read "This message is from a mailing list" and often end up with focus in the message header.
