All you have to do is disable Find my iPhone, connect the iPhone to your PC, open iTunes and in the phone section tab to Restore and press Shift + Enter. A standard Open file dialogue appears, browse to your iPSW file and press enter. You will probably be prompted that you are installing iOS 12.4.1 and probably have to press OK or Continue and then it will just go through the install the same way as if you upgrade via iTunes. Keep in mind that unless you have an archived backup done under iOS 12.4.1 you have to set up your phone as a new phone, you cannot restore a backup made in iOS 13 on a device which runs iOS 12. Also, make sure the IPSW file you downloaded is for 12.4.1 which was released on August 26, that is the only version of iOS 12 which is still supported, 12.4 or any of the 12.3 versions cannot be used any more. I find it interesting that you find the Notification Centre such a mess, on my iPhone 8 with iOS 13.1.1 it can be a little bit unpredictable at times,but for the most part it works as it should. I don’t use Braille or any hearing aids and I don’t use Flicktype any more since I prefer to practice using the slide keyboard and really the only serious issue I consider annoying is the issue when after some phone calls Voiceover and other sounds take a vacation, but I have learned how to deal with it for now and hope Apple will finally get around to it now that the public release of iOS 13 is out and hopefully everybody who has complained about it on the list has also emailed Apple Accessibility about it or called them or even better both.
