I'm wondering how to click on the create new apple id button using jaws, the
scripts, and the pc? I usually try and avoid most things via itunes on the
pc because I have such trouble using it...
If I have to use it in this case however, how can I best find what I need to
find?
Also, does the phone need to be connected while I create the new id? Do I
need to be behind the vpn when doing this as well?

I personally find it easiest to do this on a PC.
Sign out of your iTunes Store account, then at the bottom of the screen
you
should find a link to choose your store. I'm going from memory, but the
link
may be labelled with the current country you're in. Choose that link to
choose another store, and choose the UK. Search/browse for the app you
want,
and choose to buy the app. You'll be asked to create a new Apple ID, so
you'll need an email address not currently associated with the Apple ID
you
already have for the US store. When asked for a payment method, choose
none,
and the app is yours.
This is the only way to do it that will let you create an Apple ID and use
it in the store without having a UK credit card associated with it.
If you don't have a VPN provider yet, I recommend Witopia at
http://www.witopia.net.

Even if you did manage to install the app, from memory, this app
checks to see which country you're in before it plays any content, so
it would be useless without some form of VPN to the UK unfortunately.
