What Brian describes is probably not the same issue, if Voiceover goes away after a phone call with the issue which was described using a headset makes no difference. Brian, have you tried to simply raise Voiceover volume when on speaker? I would do this, plug in the headset and make sure you are on the home screen. Now unplug the headset and repeatedly press the volume up button. Do this at least 6 or 8 times and see if you start to hear Voiceover. Sometimes Voiceover volume drops to 0% for no particular reason, a headset works because the volume setting is separate from the external speaker and if that is the case simply raising the volume when on speaker will fix this.

I have seen references to a bug in IOS 13.1.2 where facetime calls make voice over go silent. This is slightly different. no sound from phone speaker. If I get a phone call, the speaker works and voice over announces who is calling and I can answer. However, after I hang up, the speaker goes silent again and there is no sound from voice over. If I plug head phones in, I get sound again. The speaker isn't broken as it works for phone calls and if I restart the phone, it works for a minute or two and then is gone again after I let the phone lock. Perhaps, this iPhone 6s is on its last legs but I see no compelling reason to get a new phone.
