I think the whole problem with this question is that what is high quality to
one might be crap to another making a realistic answer almost impossible.
The only in the ear type that I've personally tried and consider to be
excellent  from my perspective are the Etymotic 4s and 4p as well as the
Ultimate Ears model I have but can't recall the model number for.  I've also
heard great things about the Shure's that you mentioned.  Shure has always
put out really good sounding in ear phones.

In the lower price range however, the Sennheiser PX100 II with mic are
really good, open air over the ear type headphones.  These sound great for
the price.

In addition, recently, some of the one a day type sale web sites have been
selling the Incase line of headphones at a really reduced price.  I have a
pair of the Incase Pivot and a pair of the Incase Reflex headphones.  They
are both over the ear type with nice soft memory phone pads and microphone
with full iphone control.  The Pivot I've seen recently for as low as $12.99
and the Reflex as low as $20.  For this price, they are both a steal because
they sound pretty darn decent but not quite as good as the Sennheiser.

I don't use either of my in ear headphones I mentioned above much any longer
because I honestly don't like the isolation.

I think good is a relative  term. They sound way better than the old Apple
headphones but, I wouldn't want those to be my main headphones.  I find them
uncomfortable for long wear.  I have been using the Shure 215 with the
mobile kit.  These are IEM. In ear monitors.  They come with very
comfortable foam sleeves that conform to the shape of your ear.  It blocks
out a lot of noise so, I would not recommend walking with both ears in.
Thats just asking to get hit by a car.  I like that you can remove the
monitors from the cable.  I rather by a cable than replace the entire pair
of headphones if need be.  The Shure215 with mobile kit costs about $140 at
amazon.  The mobile kit is basically a cable with a mic and controls on the
right wire like the apple headphones.
