I haven’t yet had punctuation spoken that way. I wonder if it is when the source used “smart quotes” or “Smart Punctuation” which are the pretty graphical punctuation marks that screw up most screen readers. I always turn those off in Word on my computer and in Keyboard settings on my Apple devices.
