Had a similar experience and discovered that the button to turn on my Bluetooth headphones was getting pressed in my backpack. It was a absolutely terrifying, as I was standing there waiting for my Uber to pick me up and counldn't figure out why my phone wasn't talking.

This turned out to not be the case. I picked up my phone this morning to see what the weather would be, and I couldn't hear voiceover at all. I tried talking to Siri to get voiceover to turn on, but I couldn't hear any sound from SIRI. I then turned on another bluetooth speaker and it connected to my phone and I heard voiceover, but things still weren't quite wright because I still couldn't get any response from SIRI. I then thought of my headset. When I put it on I saw it was still on, and I could hear voiceover through it and I was then able to hear Siri just fine. In future if I want to use a bluetooth/wired headset, I'll turn off bluetooth on my phone.
