Had a similar experience and discovered that the button to turn on my Bluetooth 
headphones was getting pressed in my backpack. It was a absolutely terrifying, 
as I was standing there waiting for my Uber to pick me up and counldn't figure 
out why my phone wasn't talking.

Hi to all.  Last night, I decided to stop using my phone to listen to a couple 
described movies I had put on my victor stream.  So, I turned off the bard book 
I was listening to on my phone and plugged in a cable to my bluetooth/wired 
headphones.  I didn't turn off my headset or turn off bluetooth on my iPhone 
because I thought that if I plugged in my wire to my headset that my iPhone 
wouldn't be bluetooth any more and would have disconnected from my headset.  
This turned out to not be the case.  I picked up my phone this morning to see 
what the weather would be, and I couldn't hear voiceover at all.  I tried 
talking to Siri to get voiceover to turn on, but I couldn't hear any sound from 
SIRI.  I then turned on another bluetooth speaker and it connected to my phone 
and I heard voiceover, but things still weren't quite wright because I still 
couldn't get any response from SIRI.  I then thought of my headset.  When I put 
it on I saw it was still on, and I could hear voiceover through it and I was 
then able to hear Siri just fine.  In future if I want to use a bluetooth/wired 
headset, I'll turn off bluetooth on my phone.  I'm glad voiceover is working 
now and my phone is working fine.
