Hello Tara, If your friend is blind or low vision, you may suggest that he / she use the Google Voice app instead of Hang-outs. The only advantage that I know of, in hangouts, is the ability to make video calls. However, if your friend only wants to make audio calls, then Google Voice will do the job much better as it remains VoiceOver accessible. Of course, your friend will need a GV number for the best experience. Simply stated, Google Voice is a subset of Google Hangouts.
In the spirit of full disclosure, I have not attempted to use Hangouts on any of my Apple devices so there may be some good accessible workarounds of which I am not aware.
