There are the bone conduction options from Aftershokz, such as the Trekz Titaniums. Since they use bone conduction they avoid the ear altogether. They don't have the greatest sound quality, but they're good enough for screen readers, books, podcasts and the like.

The Bose Frames have speakers built into the temple of the glasses which project sound into the ear. Again, nothing goes in your ear. Sound quality is surprisingly good, but they are on the expensive side and don't have a great battery life.

Hi everyone;
As the subject line says is there a Bluetooth earpiece that does not go into
the ear canal?
I’m finding that my ears are getting more sensitive and when I put something in
my ear it bothers me.
Does anyone have any ideas?
Thank you in advance.
