I've never heard of an app disappearing without someone deleting it. I have clients who have managed to delete apps accidentally. Have you tried using the spotlight search? It could also have been accidentally moved into a folder but you'll find it by using Spotlight. Touch somewhere in the middle of the screen and swipe down with three fingers. An edit box will appear that you can type into and search for most anything on your device.
