There are several known bugs of which Apple is aware.
There are notification bugs but it is very user dependent. The last days there are many complaints not only from blind or visually impaired users; these problems are not accessibility related. For some users notifications nolonger pop up or when they do so there is no sound or some come in some do not. Sometimes problems are solved by rebooting or resetting the phone but sometimes this won’t give any help. But most users are not affected.
