Assuming you are asking for an iPhone or iPad...

Go to:
Settings/accessibility/voiceover/rotor

Here you will get alist of all of the rotor options you can add. Double tap on 
any you wish to add to your rotor. You should hear "selected" when you double 
tap an option. 
To remove a selectedoption, just double tap it again.
You can also reorder the options on the rotor to your best advantage. 

IOS will also add some context-sensitive options to the rotor depending on what 
you are doing at any particular time. For example, the "edit" rotor item will 
appear on the rotor any time you are editing information but it will not be 
there if you are not in edit mode. 

Also, apps can add their own items to the rotor depending on how they have been 
programmed. For example, the Pages app will add a number of items to the rotor 
relating to formatting options.
