I think I’ve seen this as well, but I seem to recall that just locking and unlocking the phone took care of it for me.

Actually, these devices just like our computers mess up in a variety of ways from time to time and if you have one that doesn’t, I expect you aren’t using it to its capabilities. I expect this is especially true when using Voiceover which adds another resource hit to the device. I have found that when having an issue with Voiceover using my wired Earpods that unplugging them and swiping around on the home screen a bit will get Voiceover functioning correctly again and it continues to work when plugging the Earpods back in. If that doesn’t work for you, doing a restart with the volume up and down and power buttons or powering down as you’ve been doing is probably your best option. You might also try turning Voiceover off and back on again.
