Hi Sieghard and all, When this happens to me I turn off VO and turn it back on either by Siri or pressing the power button three times on my iPhone 12 to turn it off and on.

I never have to reboot my phone in such an instance. To restore the voice, Either, press 3 fingers to the screen or turn voice over off and back on again. I guess we can fix the problem by changing the 3 finger gesture to something else. Maybe down the line the team could change it to something else that stops this from happening or we can be more careful.
