I’ve had this (similar) problem myself; any time VoiceOver encountered the word 
“defeat” it would pronounce it as “defeaturing."

I cannot now recall which voice manifested this problem, but I found that 
changing the voice remedied that particular issue.

However, it seems that no matter which voice is deployed in your case, the 
problem persists.  Have you tried adding the desired pronunciation in the 
VoiceOver utility?

Just a quick tip when adding to the pronunciation table, after selecting the 
“ADD” button you will be placed in the field that identifies the mispronounced 
word, after filling this field in you need to use the <TAB> key to 
appropriately move to the field that identifies how that word should be 
pronounced.  Trying to use the <RIGHT ARROW> key will not place focus in this 
field despite telling you that it has.  So darn frustrating!
