If you have an Apple Watch and are wearing it, you will get a notification via 
the Watch rather than spoken output from your iPhone.  That is why I turn 
notifications off for VoiceClock from within the Watch App.

If you don’t have an Apple Watch and have notifications set to anything other 
than sounds for VoiceClock on your iPhone then VoiceOver may talk at the same 
time as VoiceClock’s spoken output.  I have notifications set to sounds only 
for that reason.

Yes, VoiceClock’s speech can sound at the same times as other output without 
problems.
