I have never heard of Airplane Mode coming on by itself and I don't even think 
there is an option to set it up in Settings so it comes on at certain times, it 
always required an active action to go into Control Centre and to double tap 
Airplane mode.
As for Audio Ducking I wonder if you have it in the rotor and simply turn it on 
accidently without realizing it.
I would suggest you go to Voiceover settings, remove Audio Ducking from the 
rotor, then set it to off if that is what you want in Settings > Voiceover > 
Audio.
If you still want a quick way to access it but not have it in the rotor you can 
add it to the new Quick Settings option in iOS 15 which you bring  up with a 
2-finger quadruple tap.
