Hi, I ran into this problem when I updated to IOS 15 on my iphone 8. It turned out to be a problem with the "activity" option in Voiceover. I had an old activity which got activated that had verbosity settings that caused all punctuation to be spoken. I deleted that activity and my normal Voiceover settings took over again.
Note that in IOS 15, many more Voiceover options can be set in an "activity".
