Another answer, it is magic.
The real answer is it uses the sensors in the watch to detect a specific
pattern. Likely it will get some false positives; like when it thought I
had fallen when I was tossing laundry into the dryer....

Today, I purchased an Apple watch series 7, cellular. My previous watch
was the series 4 so this is kind of a big upgrade.
I was delighted that I was able to setup my series 7 from a backup of my
series 4. As a result, virtually all of my settings were transferred to
the
new watch.
So, I decided to give this new hand-washing timer a try. I have to admit,
it is kind of fun.
I have always considered myself to be a rather thorough hand washer but,
as
of my new purchase, I find that I need to work a little harder to get the
job done right (smile).
Anyway, the hand-washing detection / timer feature is most definitely my
first new favorite feature of Apple watch series 7.
What a wonderful tool to have onboard. Bravo, Apple.
